This is a late submission. Referrals may only be submitted after receiving approval from the president of Santa's Castle.

This form must be completed by a First Sergeant or someone of higher rank in the referee's chain of command. If there are any discrepancies in the form, the indicated POC will be contacted to confirm that the referral is legitimate. No self-referrals will be accepted.

By completing this form, you confirm that you are a member of the chain-of-command (E7 & above), a chaplain, or an approved community agency authorized to refer service members to the Santa's Castle program.
